Writing a consent letter for a minor travelling alone?

Hi, I need help on writing a consent letter for a minor ( 15 years old ) travelling alone.
The airline staff said that my parents have to write and sign it but my parents are quite busy so they ask me to write one myself and they will sign in. Since I have no idea on what to write, can anyone help me?
and Do I have to make it with a formal letter format? an example would be nice, thanks
